A completed submission MUST include:
- complete/signed physical damage application (can be printed off the website)
- Loss experience-4 years, currently valued (insurance company or MGA hard copy)
- Large losses-details of any loss in excess of $25,000
- Driver's list-include name, DOB, license number and length of service
- Current MVR for each driver listed
- Schedule of owned equipment; include year, make, serial number and current value
- Parking/security protection
- Maintenance program (copy if formalized; description if not)
- Safety program
- Historical power units, gross receipts and mileage-all for the past three years
