- Completed vehicle schedule (include use & radius, seating capacity, current values
- Drivers roster & MVR's for all drivers
- Detailed explanation and supporting documentation for all losses > $100,000
- Five (5) years of currently valued loss runs
- Current financials for accounts > $100,000 premium
- Bus Contractors Worksheet (Download from web site)
Additionally, please attach copies of the following forms and procedures if available.
- Maintenance program
- Safety Program documentation
- Vehicle service record
- Daily vehicle condition report
