A Completed Submission MUST Include:
- Completed W.F. Clayton Excess/Umbrella application (can be printed off this website)
- Accord applications -umbrella
- Completed vehicle schedule (include use & radius, seating capacity)
- Drivers roster - MVR's may be required for all
- Detailed explanation and supporting documentation for all losses
> $25,000
- Four (4) years of currently valued loss runs
Additionally, please attach copies of the following forms and procedures if available.
- Maintenance program
- Safety Program documentation
